This Salvino Sports Legends figurine features Sandy Koufax wearing his Brooklyn Dodgers uniform. The statue features a white nameplate on the front, which has a nice blue felt tip pen signature from Koufax, which is pre-certified by PSA/DNA. The statue is numbered 661/2500 on the bottom of the base and includes all of the original packaging and COA.
